HDSW Job Posting - Health Home Care Manager - Full Time

Position Overview:

To assist clients living in the community to link to necessary medical, behavioral health,

housing, social, employment, educational, and other services essential to meeting basic life

needs.

Qualifications:

BA/BS in a human services field with a minimum of two-year’s experience in a

behavioral health setting required. Must have understanding/knowledge of the behavioral

characteristics and psychosocial needs of the target population. Strong written and verbal

communication skills necessary to maintain documentation and work effectively with

clients, providers and the healthcare community.

Behavioral health, and/or forensics. Bi-lingual English and Spanish speaking a plus.

Minimum two-year’s experience in a behavioral health setting.

Must have own car, a valid driver’s license and a driving record that is satisfactory to our

insurance carrier.

Criminal History Background Check and fingerprinting required.

Salary: Commensurate with experience.

Sign on Bonus: $3000 paid out over first year of satisfactory employment
