HDSW has been involved in the evolution of community based direct-care services for vulnerable populations in NYS for almost fifty years. In today’s ever-changing healthcare marketplace, HDSW provides person-centered services to over 3,000 individuals and families each year. HDSW is a founding/owner member of Coordinated Behavioral Health Services, Inc. CBHS is a 501 © (3) organization, comprised of progressive, community behavioral health and disability service providers serving 7 Counties in the Hudson River region.
CBHS maximizes the intellectual capacity and resources of its members in order to meet the challenges of a rapidly changing healthcare system, to successfully integrate and manage care and provide the cost-effective outcome- based services required for sustainability.
