POSITION: Homeless Outreach Worker

RESPONSIBLE TO: Supervisor of HOW unit and Director of Care Management Services

ROLE: To assist homeless individuals, assessed by the Westchester County Department of Community Mental Health (DCMH) Single Homeless Assessment Center as eligible for Homeless Outreach Services and referred by the DCMH Single Point of Access, in obtaining housing and benefits, in addition to recommended treatment and community support services.

Qualifications:

* Excellent communications and interpersonal skills

* Bachelor’s Degree preferred

* Minimum two years’ experience in human services and behavioral health. Street outreach experience preferred

* Strong written and verbal communication skills

* Valid driver’s license and own car to transport clients

* Bi-lingual English and Spanish speaking preferred

Skills and Knowledge: Behavioral Health, Forensics and/or Street Outreach

Salary: Commensurate with experience.
