Care Management Services
Human Development Services has been providing care management to individuals with mental health and medical problems for over 25 years in Westchester County. We have developed expertise in providing this service and are helping over 600 individuals every year work towards their goals.
The Outreach Care management programs provide care management services to persons recovering from psychiatric illnesses so that they may maintain their stability in the community. HDSW assists individuals and families to access and coordinate needed treatment and recovery services, enrich their support system, prevent crises, and meet emergency worker needs.
The Homeless Outreach workers provide time-limited care management services to persons recovering from psychiatric illnesses who reside in Westchester shelters. The goal of Homeless Outreach is to assist the shelter resident to obtain housing benefits and other critical services.
The goal of Adult Home Care Management services is to improve mental health services for mentally ill residents living in adult homes. The service supports resident rehabilitation and recovery goals through the provision of supportive care management and support services that utilize wellness self-management approaches.
PRIVATE CARE MANAGEMENT
You or your family member will work one on one with a skilled professional to establish life goals, or maintain a comfortable lifestyle.
We will conduct a full assessment if needed to establish current goals
We coordinate and establish linkages with other medical, geriatric, or mental health providers in the community to ensure good care
We provide transportation to and from appointments
We will help you understand medical information you are given
We will talk to all doctors you are involved with if you would like assistance to coordinate your care.
We will link you to community services you are interested in
We can help you with budgeting, food shopping and laundry services
We can assist you with getting cleaning services in place
We can assist with getting help with hoarding behaviors
